Patient Billing

For your convenience we accept Visa, MasterCard and Care Credit . We deliver the finest care at the most reasonable cost to our patients, therefore payment is due at the time service is rendered unless other arrangements have been made in advance. If you have questions regarding your account, please contact us at 708 802-8300. Many times, a simple telephone call will clear any misunderstandings. At the time that you make your appointment, we will ask for your insurance information. With this information, we then can receive the estimate of the breakdown of benefits from your insurance company.

We will send you a monthly statement. Most insurance companies will respond within four weeks. All deductibles and amount that insurance will not pay, is due at the time of treatment. In some cases we can make arrangements for a payment plan. This must be done prior to the actual procedure. Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage. Your prompt remittance is appreciated.

Payment plans

Our office is glad to offer Care Credit as a payment option since this line of credit allows the patient some flexibility in choosing a plan of payment. Care Credit is a line of credit which is widely used for dental offices, hospitals, and medical offices as a payment plan. Care Credit has offered zero percent interest payment plans for a set period of time. Many patients take advantage of this economical financing which can also be used in their family doctors’ office or a hospital as needed for future use. Patients can be given a brochure to contact Care Credit directly to set up payment options in private and at their own convenience.
